Helps income-eligible Vermonters with costs related to replacing home heating systems, water heating systems, and other home appliances damaged or destroyed by the floods. Also provides rebates to upgrade home electric panels to accommodate cold climate heat pumps and additional electrification efforts. Reimbursements for eligible purchases retroactive to July 10, 2023. Also provides financial support to businesses and rental-property owners with replacing flood damaged appliances.

Provides a variety of core services including emergency support and assistance. Services include problem-solving support, assistance accessing resources, prejob guidance, employment goal setting, budget counseling, tenant-landlord mediation, emergency food services, and emergency assistance with other basic needs such as utility payment assistance. Also provides assistance with filling out benefit program applications and other forms.

Provides transportation services to non-Medicaid medical appointments, meal sites, senior centers, wellness programs, drug stores, social activities, and other healthy living related activities. Offers volunteer driving opportunities.

Helps families with children by providing case management services that support work, training and education, and cash grants for basic necessities. Reach Up helps with identifying skills, abilities, interests, training or education needs, preparation for work, childcare costs, transportation costs, and work-related costs such as clothing, tools, and supplies. Each person on Reach Up works with a case manager to create a Family Development Plan. Benefits paid through direct deposit or EBT card. Child-Only Reach Up provides cash assistance to help eligible adults care for the minor, dependent children of relatives or family friends. Reach First helps families overcome temporary, short-term financial crises and avoid the need for longer-term assistance, providing case management, financial help, and support services. Reach Ahead helps families transition from Vermont's Reach Up or Post-Secondary Education (PSE) programs to working, with supports to help maintain a job, including cash incentives for working and help paying for car repairs, work clothes, and work supplies, a 100% childcare subsidy, and a cash benefit for food.

Recruits volunteers and places them in community service jobs at non-profit organizations based on their experience, knowledge and skills. Volunteers receive training, assessment, recognition for their contributions and receive supplemental insurance coverage.

Provides community food shelf services. Personal care items may be available depending on donations.

Provides a housing trust fund for affordable housing development when funding is available, as well as information about the city's planning, zoning, and community development programs.

Provides money management counseling services that include credit counseling, debt counseling, personalized budgeting, school loan repayment counseling, bankruptcy counseling, and financial literacy training.

Provides patient-centered physical and mental health care delivered by a team of primary care and mental health clinicians. Services include medical, mental health and substance use disorder counseling, psychiatry, family planning, dietary counseling, Sexually Transmitted Infection (STI) screening, and on-site lab services. The Community Health Pharmacy program offers discounted prescriptions to all patients. Also provides consultations and support for eligibility for State and Federal Benefits and low-cost drug manufacturer discount programs.

Provides nurses and/or other trained professionals who visit the homes of women during pregnancy and after childbirth to help the women improve their personal health and development during the prenatal period and learn to care for their infant. Services include at-home assessments for expectant mothers and infants, postpartum assessments, newborn evaluations, breastfeeding support, nutrition education, parenting information, family adjustment support, skilled nursing care if needed, and assistance with coordination of family services.

Provides a full range of inpatient and outpatient care and 24-hour emergency care. Services include 24-hour physician staffed emergency room services, diagnostic imaging, general medical and surgical services, lab testing, blood supply services, hospital-based outpatient services, disease specific treatment, pain management, adult and children's services.

Provides formal supervision of offenders released conditionally on probation or parole or confined to their homes or other venues in the community.

Provides comprehensive services to individuals with developmental disabilities, autism, and other pervasive developmental disorders. Individuals are assigned professional service coordinators who are responsible for clients' Individualized Support Agreements, and for assisting them in achieving success in the community. Services include personal support and case management; specialized residential programs; psychiatric and medical care; vocational and skill acquisition training; peer support; family support services; and support services to individuals in nursing homes. Provides a spectrum of supported living arrangements to meet the needs of each person. Clinical services for individuals include provision of a psychiatrist to provide evaluation, medication checks, and after-hours consultation on medication issues. Also provides care coordination services.

Provides a safe, stable drug and alcohol-free home and supportive services with the goal of helping residents to transition to independent living and reassimilation into communities. The home environment is "family-style" where residents support one another in their recovery and cooperate to manage the household.

Provides information about preventing tax scams and fraud, as well as identity theft. Offers forms and other methods of reporting these abuses. Provides alerts about tax scams, including an annual "Dirty Dozen" list of common scams.

Provides youth employment services that include vocational testing and assessment to help identify skills, talents and experience; career guidance to help plan and map out a career strategy; tutoring and study skills training; summer employment opportunities; paid work experience including internships and job shadowing; leadership development opportunities; and support services to help overcome employment barriers.

Offers in person services including: birth control, well-person check ups, sexual health care, STD testing and treatment, HIV testing and prevention, PrEP and PEP, birth control counseling, pregnancy testing, emergency contraception, and reproductive life planning.

Provides loans and/or grants to municipalities and nonprofit organizations to purchase or expand community facilities and equipment, including public services like hospitals, libraries, fire/police stations and schools, water and waste management, and local food systems.
