Child-Only Reach Up provides cash assistance to help eligible adults care for the minor, dependent children of relatives or family friends. Reach First helps families overcome temporary, short-term financial crises and avoid the need for longer-term assistance, providing case management, financial help, and support services. Reach Ahead helps families transition from Vermont's Reach Up or Post-Secondary Education (PSE) programs to working, with supports to help maintain a job, including cash incentives for working and help paying for car repairs, work clothes, and work supplies, a 100% childcare subsidy, and a cash benefit for food.

Provides professional assistance with the assessment and development of plans for the care of individuals who, because of age, illness, disability or other difficulties, need assistance in planning and arranging for in-home care services. Hartford's community Registered Nurse will coordinate the delivery of needed services, provide assistance/education around following medical advice and proper medication taking, assess in-home mobility safety to reduce the risk of falls, assist families with understanding the issues and needs of the family member requiring in-home care services, and provide ongoing follow-up visits to ensure the best possible health outcomes. All services provided are confidential and specific to each individual's needs.

Provides on demand service without program guidelines. Passengers can request trips to anywhere in the designated service area for any reason, including shopping, medical, lunch, beauty shop, or to visit a friend. Offers RCT Rides, a mobile app for scheduling rides.

Teaches how to identify, understand, and respond to signs of mental illnesses and substance use disorders. Provides the skills needed to reach out and provide initial support to someone who may be developing a mental health or substance use problem and helps connect them to the appropriate care. Also includes information about mental health crises and risks for suicide. Separate courses are geared to different audiences: Mental Health First Aid is for adults, such as first responders, faith leaders, and caring adults; Youth Mental Health First Aid is for adults working with youth; and Teen Mental Health First Aid for high school students and youth-serving organizations.

Provides supplemental food for families with young children. WIC also provides breastfeeding support and nutrition education. Participants can get Farm to Family coupons to use at local farmers' markets.

Provides information about foster care and coordinates support and resources for foster parents of children in State of Vermont custody.

Services include primary medical care such as physical health examinations, acute and chronic health screenings, obstetric care, laboratory testing, and birth control counseling. Provides onsite coordinated primary care, mental health services, and substance abuse services to enhance access to appropriate prevention, treatment, recovery and wellness services and activities. Programs for health maintenance and public education services are also offered. Coordination of transportation to medical appointments and wellness programs is available for patients.

Aids Vermont greenhouses, farms, and orchards by assisting in the identification and control of plant pests and diseases. Callers will be instructed on how to collect, prepare, and mail a specimen of the plant or pest.

Provides air medical transportation information and referral service to help financially needy patients access long distance specialized medical evaluation, diagnosis, or treatment. Provides an initial screening of a medical patient's needs and determines the most suitable means of travel for that patient.

Provides monthly supplemental nutrition assistance benefits to low-income households to purchase food. Households in which any person is younger than 65 access their benefit using a Vermont Express electronic benefits transfer (EBT) debit card. If everyone in a household is 65 or older, or receiving Supplemental Security Income (SSI), benefits are deposited as cash into the recipient's bank account. Expedited benefits are available in emergencies and other specific situations. EBT cards may also be used to buy fresh, local products at certain farmers' markets in Vermont. A list of farmer's markets that can accept EBT cards is available on the website. In addition, 3SquaresVT benefits may be used for seeds and plants that grow fruit and vegetables at any retailer or farmer's market that accepts EBT cards.
