Collects and distributes donated winter coats.

Helps youth with disabilities plan and prepare for life after high school. HireAbility Vermont vocational counselors work with both students and school personnel. Services include career counseling, assessment, job search skills, work experiences, exploration of post-secondary education and training programs, workplace readiness, instruction in self-advocacy, and guidance and information on IEP and transition planning, 504 accommodations, benefits planning, and other needed services.

Provides social and emotional support for children with challenging behavior and/or who have or are experiencing trauma, and consultation and partnership with families and early care and education providers.

Provides community food shelf services. Personal care items may be available depending on donations. Offers ingredients for a full dinner at Thanksgiving.

Assists individuals with regaining their licenses while they are paying off fines and fees. Program participants work with diversion staff to develop contracts that include a payment plan. Participants must satisfy the Vermont Judicial Bureau and the Department of Motor Vehicles requirements. Successful completion of program leads to criminal charges being dismissed.

Provides business counseling and technical assistance to individuals who are starting, expanding, or enhancing a small business. Services include one-on-one technical assistance in business plan development, marketing materials, and financial management.

Provides holistic care services to all community members that may include the following: Nursing, Behavioral Health and Physical Health Coaching services: Nutrition education, exercise education and counseling, short-term behavioral counseling, Diabetes education, individual health coaching, medication management, weight management-individual and group support, smoking cessation, Advanced Directives, Fall Safety assessments, medical transportation and dental solutions. Community Connections Community Health Team also provides Resource Advocates who will assist with completing applications for the following services and assistance: health insurance (VT Health Connect and Medicare and Medicare D comparisons), SSDI/SSI, and prescription costs, as well as application assistance for housing, food, and utilities. The team also includes VT Health Connect navigators and assistors, as well as care managers for individuals who require support maintaining independence and stability within the community.

Provides education and support, including support of individual developmental goals, and transition planning for services after early intervention. Coaches families and early care and education providers around individual child supports.

Administers cases, according to the Bankruptcy Code and Rules, for the benefit of the public. Bankruptcy is a legal procedure for dealing with debt problems of individuals and businesses; specifically, a case filed under one of the chapters of Title 7, 11, 12, or 13 of the United States Code (the Bankruptcy Code).

Provides comprehensive medical care to individuals of all ages including physical exams, immunizations, and routine screenings, treatment of short-term injuries or illnesses, treatment of chronic illnesses, care for women during pregnancy and childbirth, family planning services, nutritional counseling, physical therapy, and other special programs. Provides preventive health care, as well as diagnosis and treatment of illness in children, adolescents and adults. Services include primary medical care such as physical health examinations, immunizations, acute and chronic health screenings, obstetric care, laboratory testing, and birth control counseling. Provides onsite integrated health services to enhance access to appropriate prevention, treatment, recovery and wellness services and activities. Pediatric primary care is staffed by specialists who are concerned with the physical, emotional and social health of infants, children and adolescents from birth to young adulthood.

HireAbility VT works closely with employment specialists at Supported Employment Programs who provide a wide range of services designed to teach necessary skills, help a person transition into a job, and ensure that employers meet the needs of their employees.

Provides individualized outpatient substance use assessment and treatment services for individuals who are dependent upon, or at risk of becoming dependent upon, alcohol or other substances. Also provides family substance use counseling for families dealing with adolescent substance use issues and provides group treatment services for adolescents. Provides information about alcohol and drug use, the effects on individuals, families and communities, as well as information to the public about strategies for prevention and early intervention.

Offers free and confidential HIV testing and counseling. Tests chiefly use a finger stick, but an oral swab is available if necessary. Results are obtained in 20 minutes.

Provides consumers with a federally mandated free credit report annually, fraud and identity theft services, and free credit marketing list removal (OptOut). Also offers a way to dispute information in a credit report.

Works with families to address obstacles or challenges regarding housing needs and assists with acquisition of stable housing. Provides support and services to maintain stable housing. This program includes individualized, family-centered case management services.

Provides a listing of Town and City Clerks, Treasurers, and County Clerks, including name, street and mailing address, telephone and fax numbers, email address, and hours. The large majority of Town Clerks are also Town Treasurers, but there is a separate list of Town Treasurers who are not also the Town Clerks.

Provides comprehensive preventive, diagnostic, and treatment services for women from puberty through the post-menopausal years. Services include well-woman exams, contraception, pregnancy and delivery services including midwifery services, pregnancy and childbirth education, as well as breastfeeding and post-partum support services. Services may also include menopausal and post-menopausal management services and urogynecology services.

Provides a wide variety of mobility products including stair lifts (sales and installation), lift recliners, motorized scooters, power wheelchairs, manual wheelchairs, walkers, rollators, knee walkers, hospital beds, patient lifts and ramps for private homes, public schools or commercial enterprises. Also provides rentals of recliners, scooters, knee walkers, wheelchairs, hospital beds and ramps, and repairs may types of assistive equipment.

Provides a victim assistance referral helpline, as well as information and helpful links to resources on crime and victimization. Offers materials on issues ranging from teen victims, survivors of violence against women, victims of stalking, crime victims considering a civil lawsuit, coping with trauma and grief, to provider trainings.

Provides health care services at all community outpatient clinics. Homebound Veterans are served by Home Based Primary Care (HBPC). Additional services can include basic nursing services, telemedicine, patient advocacy, hospice care, and physical, occupational, or speech therapies through a Skilled Home Care service.
