Provides a variety of core services in the communities it serves, including emergency support and assistance for low-income families and individuals in crisis. Provides professional assistance with the assessment and development of plans for the care of individuals who, because of age, illness, disability or other difficulties, need assistance in planning and arranging for in-home care services. The Londonderry areas Registered Nurse will coordinate the delivery of needed services, provide assistance/education around following medical advice and proper medication taking, assess in-home mobility safety to reduce the risk of falls, assist families with understanding the issues and needs of the family member requiring in-home care services, and provide ongoing follow-up visits to ensure the best possible health outcomes. All services provided are confidential and specific to each individual's needs.

Provides comprehensive services to individuals with developmental disabilities, autism, and other pervasive developmental disorders. Individuals are assigned professional service coordinators who are responsible for clients' Individualized Support Agreements and for assisting them in achieving success in the community. Services include personal support and case management; specialized residential programs; psychiatric and medical care; vocational and skill acquisition training; family support services; and support services to individuals in nursing homes. Provides a spectrum of supported living arrangements to meet the needs of each person. Clinical services for individuals include provision of a psychiatrist to provide evaluation, medication checks, and after-hours consultation on medication issues. Also provides care coordination services.

Provides youth employment services that include vocational testing and assessment to help identify skills, talents and experience; career guidance to help plan and map out a career strategy; tutoring and study skills training; summer employment opportunities; paid work experience including internships and job shadowing; leadership development opportunities; and support services to help overcome employment barriers.

Provides shelter to lost and unwanted pets, matches pets to be adopted with appropriate individuals and families, provides discounts for older pets to older adopters, and maintains information about local animal control contacts and other animal shelters to assist with finding lost pets.

Covers the costs, up to a certain dollar amount, for professional services and related costs for a simple burial or cremation for a deceased individual. Payment is made directly to the funeral home.

This peer group provides support to those who are contemplating the decision, or have made the decision to stop trying for a sibling and are finding peace with having one child.

Provides monthly take-away fresh produce and healthy groceries, as well as information/recipes for meal preparation using the fresh produce being given away at each distribution.

Works in partnership with medical centers and school districts to bring fresh produce and other food and grocery products to community members who may not have easy access to good quality, affordable food and/or rural areas where residents must travel long distances to access grocery stores that sell fresh food. Deliveries to regional sites are made monthly to partnering schools and medical centers.
