Provides professional assistance with the assessment and development of plans for the care of individuals who, because of age, illness, disability or other difficulties, need assistance in planning and arranging for in-home care services. Putney's community Registered Nurse will coordinate the delivery of needed services, provide assistance/education around following medical advice and proper medication taking, assess in-home mobility safety to reduce the risk of falls, assist families with understanding the issues and needs of the family member requiring in-home care services, and provide ongoing follow-up visits to ensure the best possible health outcomes. All services provided are confidential and specific to each individual's needs.

Provides nurses and/or other trained professionals who visit the homes of women during pregnancy and after childbirth to help them improve their personal health and development during the prenatal period and learn to care for their infant. Services include at-home assessments for expectant mothers and infants, postpartum assessments, newborn evaluations, breastfeeding support, nutrition education, parenting information, family adjustment support, skilled nursing care if needed, and assistance with coordination of family services.

Offers adult education and literacy instruction that provide learners with the opportunity to acquire the education and literacy skills they need either to complete a secondary education or to better achieve post-secondary/career goals. Provides information and applications for individuals seeking to become a Guardian ad Litem (GAL). GALs are volunteer trained advocates who represent the best interests and rights of children throughout court proceedings and with DCF, speaking for the child when necessary and ensuring they understand the judicial process and outcomes. Guardians ad Litem are appointed for children in all CHINS cases (Child In Need of Supervision): children who are abused, neglected, considered beyond the control of their parents, or truant. GALs also are occasionally appointed in other situations including delinquency, divorce, parentage, and mental health cases.

Covers the costs, up to a certain dollar amount, for professional services and related costs for a simple burial or cremation for a deceased individual. Payment is made directly to the funeral home.

Provides a spectrum of supported living arrangements to meet the needs of each person. Clinical services for individuals include provision of a psychiatrist to provide evaluation, medication checks, and after-hours consultation on medication issues. Represents Medicare beneficiaries who also receive Medicaid in Medicare appeals concerning home health services, durable medical equipment, nursing care and prescription drug coverage.

Provides rehabilitative therapies including physical therapy, occupational therapy, and speech language pathology to help individuals recovering from surgery, stroke, or illness. Also provides pediatric rehabilitative therapies.
