Assists with responding to IRS when tax filing has been unsuccessful and provides clarification and advocacy.

Enhanced Mobile Crisis Services provide rapid mobile response for individuals experiencing a mental health or substance use disorder crisis. A trained 2-person team can meet the individual at home, workplace, or another community setting, providing support and comfort including safety plans, peer support, care coordination and follow up and, if needed, temporary overnight accommodation.

Provides assistance in navigating Vermont Health Connect, Green Mountain Care and FQHC health care systems to aid clients in obtaining free or subsidized health care through referrals, including: benefit screening and enrollment assistance, dental referrals and assistance in obtaining care, patient assistance and pharmacy programs, eye care referrals, tobacco use screening and cessation program referrals, and physician referrals.

Provides primary health care, as well as prevention, diagnosis, and treatment of illness. Services include primary medical care such as physical health examinations, immunizations, acute and chronic health screenings, obstetric care, laboratory testing, and birth control counseling. Provides onsite integrated health services to enhance access to appropriate prevention, treatment, recovery and wellness services and activities. Pediatric primary care is staffed by specialists who are concerned with the physical, emotional and social health of infants, children and adolescents from birth to young adulthood.

Provides community-based supports to assist youth with successful transition to adulthood. Services include employment and housing counseling, support with finishing education, money management training, individual advocacy and connection with community resources, while at the same time providing problem-solving, positive coping and social skills training.

Provides fast track medical services to individuals who have minor medical problems that do not require complex diagnostic work, including: colds, flu, infections, muscle strains and sprains, cuts, insect stings and bites, rashes and stomach aches. Services include common immunizations, sports and school physicals, lab testing, and x-rays.

Provides comprehensive cancer-related treatment in a research-based, clinical care environment. Services include chemotherapy, radiation therapy, surgical treatment, and palliative care for individuals with cancer. Also researches cancer causes, prevention, cures, and treatments.

Collects quality food from food retailers, manufacturers, distributors, farmers, and community donors and then distributes that food to network partners, which include food pantries, soup kitchens, shelters, and senior centers.

Provides individualized, comprehensive, outpatient treatment services that address both the users' needs and the needs of those affected by the users' addictions. Groups are gender and trauma-specific and are designed to build relapse prevention skills. Provides information about alcohol and drug use, the effects on individuals, families and communities, as well as information to the public about strategies for prevention and early intervention.

Provides education and support, including support of individual developmental goals, and transition planning for services after early intervention. Coaches families and early care and education providers around individual child supports.

Provides Work Incentives Counselors who can help individuals decide whether to work, keep necessary benefits while they work, and avoid work/benefits conflicts. The program is also available to youth in high school who are in transition to adult life. Youth and their families can receive accurate information about the impact of employment on all Federal and State benefits they receive. Provides the Vermont Work/Benefits Helpline.

Provides one-on-one help and support by telephone or in person to families regarding their child's education, development, special health care needs, and transition from school to adult life. Support may include helping parents prepare for school and other meetings about their child. On a limited basis, consultants can attend Individualized Education Program (IEP), 504 and other meetings with families. Family consultants also provide information about community resources that are available to help parents and children. Also offers parent, represenative support at Act 264 or Coordinated Services Plan meetings.

Manages 6 correctional facilities throughout the state. Provides website about each facility, with visitation schedule and links to information.

Provides a drop-in community center that offers opportunities to socialize and to access support for help with acquiring supported employment, access to education and housing, peer support, and opportunities to explore holistic health and wellness. Also provides packed meals to go. During the winter months, the center offers extended hours to provide a warm space for individuals to congregate until the overnight shelter opens its doors.

Manages rental properties throughout Vermont and provides an interactive listing of its housing developments and mobile home parks. The listing is searchable by county.

Provides state income tax and related tax forms, schedules, and publications for individuals, corporate, and business entities. Forms, schedules, and publications can be picked up at the office, mailed by request, or downloaded through the website. Forms for individuals include Renters Rebate Claim, Landlord Certificate, Military Personnel, Child and Dependent Care Credit, and Extension of Time to File.

Provides a full range of inpatient and outpatient care and 24-hour emergency care. Services include 24-hour physician staffed emergency room services, diagnostic imaging, general medical and surgical services, lab testing, blood supply services, hospital-based outpatient services, disease specific treatment, pain management, adult and children's services.

Provides a twice-weekly balance and strength-training exercise program. Sessions are for up to 15 participants.

Information about local wellness programs is provided on line and is also published in flyer form.
