Also provides public advocacy, outreach, and education on fair housing issues, including publishing information, such as the Guide to Fair Housing for Families in Vermont, and conducting trainings and workshops about fair housing law.

Partners with senior meal sites and senior centers to provide hot, nutritious, noontime meals. Home-delivered meals are often available for those who are unable to reach a meal site because of illness or frailty.

Assists people with persistent psychiatric difficulties to live in the community by offering individual and group counseling, service coordination and outreach, crisis intervention services, medication management, supported housing, and employment/volunteer services.

Provides Work Incentives Counselors who can help individuals decide whether to work, keep necessary benefits while they work, and avoid work/benefits conflicts. The program is also available to youth in high school who are in transition to adult life. Youth and their families can receive accurate information about the impact of employment on all Federal and State benefits they receive. Provides the Vermont Work/Benefits Helpline.

Provides nurses and/or other trained professionals who visit the homes of women during pregnancy and after childbirth to help women improve their personal health and development during the prenatal period and learn to care for their infant. Services include at-home assessments for expectant mothers and infants, postpartum assessments, newborn evaluations, breastfeeding support, nutrition education, parenting information, family adjustment support, skilled nursing care if needed, and assistance with coordination of family services.

Provides information about area senior centers, including addresses, phone numbers, and services provided.

Offers a parenting kit, and a Milestone Tracker App that provides parents with easy-to-use illustrated checklists, tips, and activities to encourage a child's development.

A trained two-person team can meet the individual at home, workplace, or another community setting, providing support and comfort including safety plans, peer support, care coordination and follow up and, if needed, temporary overnight accommodation.

Services include 24-hour physician staffed emergency room services, diagnostic imaging, general medical and surgical services, lab testing, blood supply services, hospital-based outpatient services, disease specific treatment, pain management, adult and children's services.

Provides MAT (medication-assisted treatment) for adults struggling with opioid use disorder. Provides assessment and treatment for opiate addiction. Services include screening intake, assessment, treatment planning, methadone maintenance and medically supervised withdrawal, counseling services, case management and referral, medical services for opioid disorder, discharge planning, and follow-up.

Provides a visor card to help individuals who are Deaf or hard of hearing communicate with law enforcement officers if they are pulled over while driving. There are tips for how officers should communicate with the cardholder, followed by options that cardholders can point to indicating how best to communicate (writing, texting, lip-reading, etc.) with them. On the back, there are graphics with one-word labels that officers can point to under “I need to see your” (license, registration, insurance, other) and “Violations.” The last section is “Help” and lists options for cardholders to show the officer what they need (medical, lost, tow, etc.).
