Provides physical therapists at request of physicians to do growth and development assessments, to establish exercise routines, to monitor progress, and to assist families with obtaining adaptive equipment.

The UVM Health Network Home Health & Hospice is a non-profit, Medicare-certified, home health care agency delivering services to residents of Chittenden and Grand Isle Counties. Care is provided to adults who need in-home assistance, rehabilitation, long term care, or end-of-life care, as well as to critically ill children and to vulnerable young families. UVM Network Home Health & Hospice also promotes the general welfare of residents through health promotion outreach and community wellness programs. Services include Home Care, Hospice Care, Palliative Care, Long Term Care, Rehabilitative Therapies, Maternal Child Health Services, Pediatric Nursing, and Private Duty Nursing Services.
