Provides comprehensive, coordinated long-term care in an individual's home. Services include case/care management, homemaker assistance and assistance with the activities of daily living, including personal care and respite care that enables frail elderly and permanently disabled individuals to live safely, comfortably and as independently as possible at home. Also provides Medicaid Waiver, community-based one-to-one long term care services to individuals with Traumatic Brain Injury.

Central Vermont Home Health and Hospice is a non-profit, Medicare-certified home health agency delivering services to Central Vermonters of all ages. Care is provided to adults who need in-home assistance, rehabilitation, long term care, or end-of-life care, as well as to critically ill children and to vulnerable young families. Services include Home Care, Hospice Care, Palliative Care, Long Term Care, Rehabilitative Therapies, Maternal Child Health Services, Pediatric Nursing, and Private Duty Nursing Services.
