Provides preventive health care, as well as diagnosis and treatment of illness in children, adolescents and adults. Services include primary medical care such as physical health examinations, immunizations, acute and chronic health screenings, laboratory testing, and birth control counseling. Provides onsite coordinated primary care, mental health services, and substance abuse services to enhance access to appropriate prevention, treatment, recovery and wellness services and activities. Programs for health maintenance and public health awareness services are also offered. Coordinates and supports provision of non-emergency medical transportation through local transportation programs to health and wellness appointments.
