Provides primary and preventive health care, as well as diagnosis and treatment of illness in children, adolescents and adults. Services include primary medical care such as physical health examinations, acute and chronic health screenings, obstetric care, laboratory testing, and birth control counseling. Provides onsite coordinated primary care, mental health services, and substance abuse services to enhance access to appropriate prevention, treatment, recovery and wellness services and activities. Offers coordination of transportation to medical appointments and wellness programs for patients. Programs for health maintenance and public health awareness services are also offered.
