All forms for documenting health care decisions can be downloaded from the VEN website at no charge. Individual copies of the Taking Steps booklet are available free of charge from Vermont Ethics Network.

Vermont Ethics Network (VEN) is a local non-profit organization working to advance ethics in health care. VEN provides education, training and resources to health care professionals and community members on medical ethics, health care decision-making, advance care planning, advance directives, limitation of treatment orders (DNR/COLST), palliative and end-of-life care, and Vermont's law on medical aid in dying (Act39).
