Provides physical health exams, acute and chronic health care, immunizations, women's health screenings, limited mental health counseling by appointment, lab and blood work, x-rays (Castleton and Mettowee only), and assistance with finding affordable prescription medications. Provides patient-centered physical and mental health care delivered by a team of primary care and mental health clinicians .The clinics also provides referrals to other health and non-health services, and will screen patients and callers for eligibility for other health services and public insurance programs, as well as assisting with the application process. The Brandon, Castleton, and Mettowee clinics also hold diabetes education classes. This location also serves as an Express Care urgent care clinic.

Community Health Centers of the Rutland Region (CHCRR) is part of the Federally Qualified Health Center (FQHC) system of clinics. CHCRR operates four primary health care clinics throughout Rutland County and one in Addison County, a pediatric clinic in Rutland City, ExpressCare facilities in Castleton and Rutland, and dental clinics in Rutland City and the Town of Shoreham.
