Provides confidential consultations by trained professional case managers and pre-dismissal hospital assessments to determine eligibility for a variety of programs including Medicaid, SSI, food stamps, fuel assistance, housing, transportation, state pharmacy assistance programs, Lifeline telephone credit, respite and in-home services. Provides assistance with filling out forms for submission. Case managers also work with the VA in assisting veterans with applying for the Veterans Independence Program.

Central Vermont Council on Aging is a non-profit senior services organization that plans, develops and coordinates comprehensive services for older adults in support of their independence, self choice, and well being in Washington, Lamoille, and Orange Counties. Services include, but are not limited to, information and referral assistance, case/care management, nutrition services, healthy living programs, health insurance counseling, hoarding issues, and support for caregivers. CVCOA also serves as the umbrella agency for RSVP of Central Vermont and the Northeast Kingdom.
